If you have experienced an incident involving personal injury or property damage that you believe may involve the City of Thorold, you may submit an Incident Report for review.

This form collects important details about your incident—including the date, location, description, estimated damages, and any supporting documentation—to assist in the review process. Please ensure all information is complete and accurate to support a timely review. Once submitted, your report will be forwarded to the City’s insurance adjuster, who will follow up with you as needed.
